Further to the consultation, if treatment is pursued, the following additional information is taken:

 

– Treatment consent forms

– Clinical photography- these will be uploaded to your own personal record on a secure electronic database that can only be accessed securely with personalised PIN access by current employees

– Treatment details- these will be in hand written form written by the practitioner of each individual treatment and stored in secure locked filing system.

– Appointment details (times and treatment titles) – this information will be stored on electronic database that can only be accessed securely with personalised PIN access by current employees.

– Payment records – (strictly only date, amount and method) – this information is stored on your own personal record on the electronic database and on your hand written clinical notes stored in secure filing system.

 

The additional information gathered following the point of consultation will not be shared with any third parties except in such instances during the treatment process where it is necessary, such as:

 

– Pharmacies (prescriptions)

– Insurance company (claims)

– Legal personnel (prospective legal proceedings)

-Other medical professionals if requested by you for additional treatments or referrals

 

 

(c) Information relevant to customer surveys, competitions, social media and special offers.
